2025 FEVA Mini-Club Commitment Form

All commitment forms filled out by August 1 will receive a $50 discount for the mini club tuition (regular price $550, discounted price of $500). This form will collect a $250 deposit. By signing, I am committing for my child to join Florida Elite Volleyball Academy, ("Club"), for the mini club season. I understand that my pyament is non-refundable. I also understand and agree that by signing this commitment form, in the event my child quits, decides to discontinue playing, or attempts to join another Club, I will remain responsible for paying the entire amount due for the season, as set forth more fully in the FEVA Parent Handbook, as well as any legal fees necessary for the Club to enforce this Agreement. If my child is injured and unable to complete the season, I will remain responsible for all fees due unless the Club, in its sole discretion, agrees to waive such fees. I have reviewed the fee schedule and understand that I must pay all fees by the due dates regardless of the duration of my child's participation.
